LOST, STOLEN or DAMAGED PASSPORT

Step (1)

Collect Documents

- Complete online application and gather required documents below. 
- We will walk you through each step of the way.

Step (2)

Email Documents

- Email in all documents for review to info@skyembassy.com
- We will pre-check and verify all documents are correct for submission.

Step (3)

Personal Appearance 

- You will visit your local passport agency/post office.
- A federal agent will review the documents and instruct you to verify and sign your application. 
- The federal agent will then seal the envelope and hand it back to you./post office.

Step (4)

Ship/Drop Envelope

- You may either ship in or drop off your envelope at our location with an appointment.
- We will submit it to the U.S Department of State the next business day for processing.

Step (5)

Receive your new Passport

- Once your passport has been issued we will contact you immediately! You may either pick up your new passport or have it shipped back to you.

IMPORTANT - MANDATORY PERSONAL APPEARANCE TO A LOCAL FEDERAL ACCEPTANCE AGENCY IS REQUIRED FOR THE FOLLOWING:

  • Adults with lost or damaged passports.

  • Must turn in expired, damaged or mutilated passport to The U.S Department of State.

  • ALL expedited passports MUST be typed. No hand written applications will be accepted. 

Required Documents

1- UNSIGNED DS-11 Form

  • DS-64 Form (IF REQUIRED)

  • Complete your ONLINE application form: CLICK HERE

  • Please CLICK HERE for IMPORTANT instructions on how to fill out your online application. 

2- Required Documents

  • MUST provide your  original birth certificate, certified copy or original naturalization certificate.

  • MUST provide Name Change documentation, if applicable (Original marriage certificate OR court order documentation) 

  • IF you cannot obtain one of these required documents, The Department of State can do a FILE SEARCH service for $150. 

3- Proof of Identification 

  • MUST provide your VALID driver’s license or Federal State ID card

  • IF you do not obtain an ID card:

  1. You must visit your local DMV and apply for either a driver license or State ID card. Please provide the DMV temporary print out as proof that you have applied for an ID.

  2. REQUIRED WITNESS: During your personal appearance at the Post Office you must bring a witness you have known for over 5 years and  can provide proof of identification. Witness can be a friend, family member, but cannot be a co-worker or employer/employees.   Please note: Your witness will sign a witness form.

5- Travel Itinerary

  • Must be a round trip itinerary printed from an online vendor or on a travel agency letterhead.

6- Passport Type Photos

  • Two 2x2 passport type photographs

  • Photo MUST be taken within the past 6 months 

  • Neutral expression and photo must be in high quality 

7- Payment

  • ONLY cash, check or ATM card is accepted at the acceptance agency (credit cards are NOT accepted) 

  • Processing service fees may be paid by cash, check, debit/credit card and money orders directly to Sky Embassy. You may make the payment in person by visiting our office, by mail, or by phone.
