Application must be completed on a laptop or desktop and printed out to sign as it may not be supported on any other device.

NO hand-written forms can be accepted. The application form MUST be completed ONLINE and typed in all caps.

 SOCIAL SECURITY NUMBER must be included and correct as it can delay/reject passport issuance. 

Enter FULL NAMES including your middle names as it appears on your previous passport or legal documents. In failing to do so this will cause delays/rejection/suspension by the Department of State.

Do not abbreviate, do not include nicknames, or titles to your name. Name must appear identical as in previous passport/legal documents.

Once you have completed your online application form, print it and DO NOT SIGN THE FORM as this step is very important and must be done in the presence of the federal agent.
